Probobly a glitch, you should do Insane again, lol that would yes be annoying and frustrating, but again its a glitch, I can't think of any other reason for that to happen.

i am baffled, i'm going through this game on insane, it's so hard, anyway i finally finish it..... no achievement.... i look on my stats and see this is the only chapter not completed on insane, i know myself i started and finished on insane and looking back i cant see anything i have missed etc. any idea why this has happened anybody>?

You get that achievement easy during the driving part with the turrets when you get stuck in the stand
